146. Piver II Versus Piver III in the Treatment of Cervical Carcinoma

Summary: We reviwed the clinical files of 48 patients with cervical cancer treated at Gynecologic Oncology Service -St.Raffaele Hospital- University of Milan from November 1992 till November 1996. In 17 cases Piver II Radical Hysterectomy was performed while in 31 cases Piver III tecnique was adopted. The mean age of the patients was 56.2 (range 34-81) in the Piver II cases and 46.3 (range 29-71) in the Piver III patients. Mean number of limph nodes removed was 12 in Piver II (range 4-25) and was 16 in Piver III cases. Positive nodes were found in 6% of Piver II versus 29 of Piver III cases. In 6% of Piver II and in 3% of Piver III vaginal margins of resection were involved. Major postoperative complications occurred in 5 patients (16%) submitted to Piver III procedure( 2 bowel obstructions and 2 postoperative bleedings requiring relaparotomy; 1 case of ureteral fistula was cured by double J application ). No major postoperative complications occurred in Piver II population. Disease free survival of patients submitted to Piver II is 94% versus 81% of Piver III cases. Good clinical selection permits the modulation of surgical radicality in patients treated for cervical cancer.
