199. Perineal Ultrasound Evaluation after TVT (Tension Free Vaginal Tape) Operation.

Summary: Ten patients underwent preoperative urodynamics investigations: 6 were affected by genuine stress incontinence (GSI) class IV A and 4 class IV B stress test (Ferrari classification). All patients were submitted to TVTwithout other surgical procedure associated from March to October 1999. Immediate postoperative perineal ultrasound evaluation has been performed in a supine position. We measured the difference between the urethral meatus-vaginal tape distance and  vaginal tape-bladder neck distance at rest and the distance between the vaginal tape and the lower edge of pubic bone at rest and during Valsalva maneuver. We repeated the same measurements after 3-6 months from the operation (mean 5.7). We also compared the findings with the postoperative outcome for GSI. The vast majority of patients (80%) was clinically cured, 1 improved and 1 was unchanged. The distance between vaginal tape and the lower edge of the pubic bone at rest was 17.2 mm (mean value) in the cured patients, 13 mm in improved woman and 17.1 mm in unchanged case. Longer follow-up and wider survey will help to prove the reliability of Perineal Ultrasound Evaluation in TVT cases.
